Description:

This table contains free-text comments describing why a service request for Continued Care and Services Coordination is being considered or declined when the reason '99 - Other (comment)' is selected. This will have the free-text follow-up comment when the service request status is considered or free-text reason comment when the service request status is declined.

	The date of this contact in calendar format.

	The Community ID (CID) of the instance that owns this contact. This is only populated if you use IntraConnect.
7	RSN_COMMENT	VARCHAR	No
	Free-text response for a provided reason for the patient's status for a reason of 'Other (comment)'. This will have the free-text follow-up comment when the service request status is considered or free-text reason comment when the service request status is declined.
